Caractérisation des Arthrites septiques à l’Hôpital national Ignace Deen, Conakry république de Guinée

Introduction : L’arthrite septique se définit par la présence de microorganismes vivants au sein de l’articulation. Elle est habituellement mono articulaire, mais peut parfois concerner plusieurs articulations. L’objectif de notre étude était de décrire les caractéristiques épidémiologiques, cliniques et paracliniques des arthrites septiques à l’hôpital national Ignace Deen de Conakry, république de Guinée.Patients et méthode : Il s’agissait d’une étude rétrospective descriptive incluant les patients atteints d’arthrite septique et admis à l’hôpital national Ignace Deen sur une période de 3 ans. La confirmation bactériologique de l’arthrite septique a été définie comme la mise en évidence d’un germe par examen direct, culture dans un prélèvement de liquide synovial. Nous avons exclu les arthrites septiques sur prothèse ou autres matériels intraarticulaires. Résultats : Nous avons colligé 44 observations d’arthrites septiques dont 31 hommes (70%) des cas avec un âge moyen de 56 ans (extrêmes 27 et 80 ans). Au moyen un facteur de risque a été identifié chez 31(70%) patients. Les formes monoarticulaires étaient les plus fréquentes 40 (90%) des cas avec une atteinte prédominante des genoux. A l’examen bactériologique du liquide articulaire, le staphylococcus aureus 24 (54%) était le germe le plus fréquent.Conclusion : L’arthrite septique représente une urgence diagnostic et thérapeutique. Elle survient préférentiellement chez les personnes avec facteur de risque. Devant l’endémicité des germes responsables en Afrique noire, le plateau technique doit être amélioré pour éviter tout retard diagnostique.Mots clés : Arthrite septique - Staphylococcus aureus - Guinée.
